    • Undergraduate Reactivation/Petition for Readmission

    1. Navigate to the 'Campus Connection Guest page'
    2. Click on the 'NDUS eForms' tile (as shown below).
    3. Click on 'North Dakota State University' and then 'Reactivation Petition' from the left side menu.
    4. Complete the form and monitor the email address entered on the submission for any communication about the completion of the form.
